Part 93228pb01 is the gorilla mask headgear worn by the Gorilla Suit Guy from 8803 Minifigures Series 3. It has dark grey printing for the face, as well as nostrils and a mouth, and two holes for the eyes on the Minifigure head this mask is put on. However, the eyes need to be evenly spaced to be seen through the holes in the mask.
